Relative Lack of Culprit and Obstructive Coronary Lesions in Patients With Acute Ischemic Stroke and Elevated Cardiac Troponin.

چکیده
1228 The measurement of cardiac troponins (cTn) is widely used to diagnose myocardial injury or necrosis, and has become an important tool for the risk stratification and clinical management of patients with acute coronary syndrome (ACS). However, elevated cTn levels are also found in nonACS patients. For example, in a recent study of patients with chronic stable coronary artery disease, higher cTn levels were observed in patients with concomitant kidney disease or diabetes mellitus. Furthermore, ≈15% of patients with acute ischemic stroke have elevated cTn, many of whom have no other clinical manifestation of acute coronary ischemia. Because cTn still provides important prognostic information regarding mortality outcomes in patients who have had a stroke, current American Heart Association guidelines recommend its measurement in all patients presenting with acute ischemic stroke. However, the role of cTn in predicting culprit or obstructive coronary lesions in these patients remains unknown, but may be important to ascertain if subsequent revascularization therapy could be shown to be beneficial.

Coronary Angiographic Findings in Acute Ischemic Stroke Patients With Elevated Cardiac Troponin: The Troponin Elevation in Acute Ischemic Stroke (TRELAS) Study.
BACKGROUND A relevant proportion of patients with acute ischemic stroke (AIS) have elevated levels of cardiac troponins (cTn). However, the frequency of coronary ischemia as the cause of elevated cTn is unknown. The aim of our study was to analyze coronary vessel status in AIS patients with elevated cTn compared with patients presenting with non-ST-segment-elevation acute coronary syndrome (NST...
متن کاملTroponin elevation in acute ischemic stroke (TRELAS) - protocol of a prospective observational trial
BACKGROUND Levels of the cardiac muscle regulatory protein troponin T (cTnT) are frequently elevated in patients with acute ischemic stroke and elevated cTnT predicts poor outcome and mortality. The pathomechanism of troponin release may relate to co-morbid coronary artery disease and myocardial ischemia or, alternatively, to neurogenic cardiac damage due to autonomic activation after acute isc...
متن کاملComparison of pregnancy-associated plasma protein-A, troponin and creatine kinase-MB levels in acute coronary syndrome
Background: Early diagnosis and proper treatment of patient with acute coronary syndrome (ACS) and ischemic heart disease are important in determining prognosis, preventing adverse effects, and may even save lives. In this study, the level of pregnancy-associated plasma protein-A (PAPP-A) in ACS patients was compared with the control group, in addition to cardiac Troponin (cTn) and creatine kin...
متن کاملPlatelet count and MPV, routinely measured but ignored parameters used in conjunction with the diagnosis of acute coronary syndrome: single study center in Iranian population, 2010
Background: Myocardial infarction is the major cause of morbidity and mortality in industrialized countries. Platelet count and the mean platelet volume (MPV), a simple and reliable indicator of platelet size which correlates with platelet activation, might associate with troponin in acute chest pain. Methods: We analyzed MPV of 851 patients who were admitted to Rasoul-e-Akram Hospital with ac...
